  • Job Posting: RN Case Manager
  • Schedule: Monday-Friday 7:30-4:30 (60-minute lunch)
  • Weekends: 1-2 days/month (based on need - these will be -- day shifts)
  • This position will primarily be on-site, with travel required between various hospitals, ambulatory locations and administrative buildings.
    • Job Requirements
    • 5 years clinical experience in an acute care setting, preferably in a med/surg
    • *BCLS certification required*

        • Unit Specific Information
        • Facilitates patient screening and enrollment into the Hospital Care at Home program
        • Performs assessments of patients physical, functional, psycho-social needs including home environment
        • Educates providers, nurses, case managers, social workers and other care team members on the Hospital Care at Home program and its benefits to patients
