Job Title: Registered Nurse Home Health Discharge PlanningJob Summary:We are seeking a compassionate and skilled Registered Nurse to join our team as a Home Health Discharge Planning Nurse. As a key member of our care coordination team, you will play a vital role in ensuring seamless transitions of care for our patients.Responsibilities:

• Determine home care eligibility and review patient insurances and medical documentation.
• Coordinate health care services as ordered by the attending physician.
• Assist hospital/facility personnel in the discharge planning process.
• Ensure coordination of all ancillary services per the patients' needs following discharge.
• Promote well-being of patients.
• Build and maintain lasting positive relationships with patients/clients and facility/hospital personnel, physicians and other team members.Requirements:
  • Registered Nurse with current license in the state of employment.
  • Minimum of two years of experience. Home care experience preferred.
  • Valid driver's license and auto insurance in your name as a driver.
