        • Required if candidate is using a compact license from any state other than Ohio.
        • Check UD/Unit Cost Center Link to determine certifications required per unit.
        • AHA only (Instructor/Provider and RQI acceptable). Letter of completion not acceptable.
        • Travelers can renew ACLS, BLS, PALS at the facility.
        • Required for ER, ER OB, and ER Psych.
        • Required for L&D, Post-Partum, and OB.
        • Surgical Technologists: National cert not required.
        • SPT: IAHCSMM or CBSPD certifications are preferred, not required.
        • Required annually: HIPAA Safety Manual Acknowledgement.
        • License Verification required within 30 days of start.
        • Required for RNs, LPN/LVN, APs.
        • NIH required for ER and all Critical Care RNs (ICU, MICU, SICU, Neuro ICU, CVICU, and Critical Care Float).
        • Check UD/Unit Cost Center Link to determine if required for any other units.
        • All RN/LVN candidates must complete Medication Test/Pharmacology Competency Exam prior to start.
        • Unit-Specific Competency Exam Required.
        • For all new contracts: Dysrhythmia/EKG Exam required for ALL RNs WITHIN 1 YEAR OF START - must score a minimum of 80%.
